This microcannula is blunt-tipped, making puncturing vessels very difficult and injecting much safer. This technique leads to less pain, bruising, swelling, and injury, resulting in less downtime and dramatically reducing the risk of complications.

How Under-Eye Fillers Work

The reason we and many other filler experts rely on hyaluronic-acid-based fillers centers around the incredible moisture-retaining capability of this molecule. Hyaluronic acid, which is naturally produced by the body, can retain roughly 50 times its own weight in water. To provide some context, that’s around ten ounces of water for every one gram of hyaluronic acid. As such, it only takes a small amount to give your under-eye areas, lips, cheeks, and common problem spots for fine lines a hydrating and moisturizing volume boost.

Especially if you intend to undergo filler treatment in other areas, it’s important to understand that there are several types of formulations currently available on the market, each of which can treat different issues. For example, fillers formulated with high-density, large particles and more cross-linking are typically more appropriate when working with deeper tissues. Fillers with smaller particles and less cross-linking are better for fine lines and other more superficial skin concerns.

As for how long it takes the body to safely metabolize the hyaluronic acid once injected, this varies according to a range of factors, including the amount injected, the type of filler, the patient’s metabolism, aftercare and follow-up visits (more on this below), and various additional factors. To provide a broad range, the results of dermal filler treatment can range from six months to as long as a year and a half.

It’s the thinnest layer of skin on your body and one of the main reasons it’s susceptible to dark rings. What to Expect From the Treatment Process

Every treatment plan must begin with a personalized consultation. The purpose of this initial visit is to determine your eligibility for under-eye treatments, which requires carefully assessing a number of areas. For example, we will examine your under-eye area as well as your surrounding facial features to determine whether or not you’re a fit candidate for fillers.

We will also discuss your medical history, your goals, and any concerns or questions you have before developing a treatment plan if you are eligible. In some cases, consultation and treatment may take place on the same day. 

In either case, when it’s time for the treatment itself, the first step once you are comfortably positioned is to apply topical numbing cream, if applicable. Then, the injection will be administered via the blunt-tipped microcannula mentioned above, lowering the risk of a vascular injury. 

Typically, this is a quick and comfortable treatment, often taking as little as 20 or 30 minutes in total. You may feel a slight pressure or a feeling of fullness throughout the treatment. Your injector may massage and/or shape the filler once injected to ensure natural-looking results while allowing it to perfuse throughout the treatment area more effectively.

Once the treatment has concluded, the area will be assessed to ensure an appropriate response to the injection(s), and shortly after, you will be cleared to carry on with your normal schedule. As we will cover in more detail below, mild tenderness and swelling is to be expected for one or more days following your session, after which your results will become more visible. 

How to Know if You Are an Ideal Candidate for Tear Trough Fillers

First, we’ll review the factors that make someone an ideal candidate for tear trough fillers, and then, we’ll cover some issues that may negatively impact your eligibility for this treatment.

Tear trough fillers are for people who have experienced mild to moderate volume loss under the eyes that creates a hollow (and usually darkened) appearance. If you have experienced these issues, often accompanied by fine lines that create a tired look, you may be an appropriate candidate for tear trough fillers.

While tear trough fillers are a safe and well-tolerated non-surgical undereye treatment that can produce results for patients wondering how to get rid of dark circles, there are still some medical factors that may affect your candidacy. For example, patients with known allergies to the filler itself, the lidocaine used in some fillers, or any other agents contained in the formulation(s) are not qualified for this treatment.

Additionally, patients with active skin infections in or around the treatment site, skin conditions, certain autoimmune or bleeding disorders, and a variety of other conditions (like heart disease or metabolic disorders) may not be eligible to undergo this treatment.

Not everything in the above list is guaranteed to disqualify you from tear trough filler treatment. In a consultation, we will discuss your medical history and other relevant factors in order to clearly establish your eligibility for treatment.

Expectation Management: What Fillers Can and Can’t Do

Even for patients who are otherwise eligible to receive tear trough filler treatment, if expectations aren’t aligned with the results that this treatment can realistically provide, then it may not be the most appropriate option.

As a non-surgical undereye treatment option, tear trough fillers can provide a noticeable enhancement to the under-eye area, but still a subtle and temporary one. Tear trough fillers are meant to restore your natural features, not transform them. This includes brightening and smoothing out the under-eye area to give you a more alert appearance. You should see a less dramatic contrast between the under-eye area and the cheek.

Still, this doesn’t mean that under-eye fillers will completely eliminate the dark circles under your eyes. Oftentimes, this darkness is caused by blood vessels or pigmentation, neither of which are effectively corrected by dermal filler treatment (though there are other non-surgical undereye treatment options like topicals and lasers that can help). 

Also, it’s important to understand that tear trough fillers are not meant to treat baggy or puffy under-eye areas. The premise behind filler treatment is to restore lost volume, which would be counterproductive when applied to baggy or puffy eyes. Depending on the cause(s) of the issue, lower blepharoplasty surgery or certain non-surgical undereye treatment approaches may be more appropriate in this case.

Finally, tear trough fillers do not eliminate the need to optimize your sleep schedule and skincare routine. This treatment is not meant to replace your wellness routine, but to be integrated into it as one tool among several. 

Risks and Side Effects of Under-Eye Fillers

First, let’s cover the much more common side effects that most patients can expect to encounter.

As mentioned, mild to moderate swelling around the injection site is a very common occurrence immediately following treatment. This typically resolves within a week, and rarely requires more than a cold compress to manage. Alongside swelling, many patients experience bruising, tenderness, and soreness, which also tend to resolve within a few days or approximately one week.

You may also notice some very small bumps, and/or texture irregularities affecting the treatment area soon after your session. Don’t be alarmed, as this doesn’t mean the treatment was ineffective or botched. This is a common occurrence that resolves itself as the filler settles into the tissues. Providers aim to prevent this by massaging the filler into the skin immediately after treatment.

Uncommon side effects associated with tear trough filler treatment include swelling that persists for longer periods of time, a “blushing” appearance known as the Tyndall effect, and migration of the filler to nearby areas. Finally, very rare, but potentially serious risks include vascular injury, resulting visual impairment, and infection.

Tear Trough Fillers vs Alternative Treatment Options

While fillers are without a doubt one of the most popular approaches, if not the most popular approach for improving under-eye hollowing and dark circles, they certainly aren’t the only non-surgical option available to patients today. 

Laser treatments, PRP therapy, and topical products are among the most popular alternatives that patients turn to in order to achieve similar results. Each treatment approach comes with unique advantages and disadvantages.

Laser treatments, for example, are typically a more suitable option when the dark under-eye circles are attributed to hyperpigmentation. However, lasers cannot restore volume like dermal fillers can. Given their complementary effects, many patients opt to combine both of these treatments (both for under-eye circles and for various other concerns). 

PRP, or platelet-rich plasma therapy, involves the concentration and reinjection of your own blood’s platelets to stimulate a number of key healing mechanisms, including collagen and elastin production. This can help to flatten puffy bags, lighten dark circles and smooth fine lines, but like laser treatment, PRP is typically not as effective as fillers at restoring volume loss.

Finally, topical products can brighten, firm up, and “de-puff” the under-eye area, but this comes with an important caveat: be very careful to only use well-reviewed products that contain proven ingredients. Retinol, caffeine, peptides, and vitamin C can help brighten under-eye areas to an extent, but for more advanced cases, a procedure may be more effective.

Preserve Your Results With Aftercare and Maintenance

Whether you rely solely on dermal fillers or if you take a more integrated approach to restoring your under-eye areas, proper aftercare practices can make a major difference in both the quality and longevity of your results.

Since each case is unique, it’s important to defer to your provider’s specific aftercare instructions. With that in mind, aftercare recommendations immediately following your treatment (first 1-3 days) typically include application of cold compresses, sleeping with your head elevated for at least one night, and avoiding touching the area. It’s also important to avoid alcohol, vigorous exercise, and high-heat environments (sauna, hot tub, etc.) for at least 1-2 following treatment.

After the acute recovery phase is over, wearing sunscreen on a regular basis, staying hydrated, and maintaining a regular sleep schedule can all help to prolong your results. Between these practices and regular follow-up visits spaced 9-12 months apart, you can keep your results looking fresher for much longer.
